(Initial visit in person only) • New Patient & Renewal Visits We welcome new patients, provide 7-month recertifications, and offer seamless transfer of care from another physician. • Veteran Discount Program Veterans receive a reduced rate for their initial certification or renewal with valid proof of service. • OMMU State ID Assistance Our team helps all qualified patients complete the Florida Medical Marijuana Use Registry (MMUR) application and obtain their state-issued ID card. • Secure Online Patient Portal Easily complete intake forms, upload required documents (e.g., Florida ID or proof of residency), and manage your appointments anytime. • Mobile Access Our portal is fully accessible via phone, tablet, or desktop to make the process easy and convenient. • Multilingual Services We provide support in English, French, Spanish, to ensure accessibility for all patients. ⸻ Service Area MedLeaf Clinic is located in Dania Beach, Florida, and proudly serves Broward County and surrounding communities including Fort Lauderdale, Hollywood, Hallandale, Aventura, and beyond.
